Jack is survived by his wife of 37 years, Barbara Lizana; daughters, Barbara Irick (Timothy) and Krista Addington (Michael); three grandchildren; one great-grandchild; sister, Evelyn Marie Lizana; and many extended relatives and friends.
Interment will take place at Albert G. Horton Memorial Veterans Cemetery at a date to be determined.
